THE AFTERCARE PROGRAM

The Credo Aftercare Home, established in 1977, serves as a discharge option for the residents completing high levels of care. Referrals are accepted from Credo residential programs, as well as other programs outside of the agency, for individuals seeking a sober group living situation that encourages responsibility. It provides a family atmosphere that promotes individuality, healthy decision making and improving self awareness. This is made possible through peer support, individual treatment planning, and group and individual counseling. The Aftercare residence can house 15 individuals, and the age requirements are 16 years and older. Also, it is a 6-12 month program with variable lengths of stay depending on the individuals needs.

Residents are expected to share in household duties including meal preparation, house work and lawn maintenance. Residents are required to become employed and work a minimum of 20 hours weekly and are encouraged to explore, and when ready to pursue, educational/vocational opportunities that range from GED to college or vocational training. Various avenues of self-help in the community are also encouraged.

House group is held once per week to allow residents to process accomplishments and difficulties in day to day living, in order to prepare their selves for the primary goal of sober independence. Outpatient counseling services are provided at the Credo Community Center Outpatient Clinic located at 595 W. Main Street. Contact is maintained with legal entities.

Residents contribute 30% of their gross wages to their shelter expense. The balance of the program cost is covered by Public Assistance, however if they are working for pay the Department of Social Services will not allot them personal needs money, expecting them to earn it for themselves. Anyone not eligible to receive Public Assistance is put on a sliding fee scale.

The Aftercare serves as a re-entry point for residents nearing completion of the Credo Intensive Residential Program. They spend 1-2 nights a week at the Aftercare, joining in the meetings and activities to help them transition from treatment back to the community.

Referrals are received from the Credo Intensive Residential Program, the Men’s Half-way House and other sources such as outpatient clinics, other programs, probation, courts, and self referrals.

This is a Congregate Care Level II program. Any individual not 
qualifying for DSS funding will qualify for a sliding fee scale.
