POSITION VACANCY NOTICE

Licensed Practical Nurse :

Date: July 30, 2010
Job Title: Licensed Practical Nurse
Status: Non-exempt/Hourly
Program: Residential
Location: Women’s Intensive Residential Program
Shift: Could include days, evenings, nights, weekends, and/or holidays
as specified, available for On-Call work as assigned
Report to: Program Director of Women’s House

Description: The Licensed Practical Nurse will oversee the day-to-day medical needs
of the Intensive Residence. The RN is responsible for intakes, referrals, and coordination of all medical planning and implementation. This position will provide medication control, schedule medical appointments as needed, direct
first aid when necessary and train and direct staff in all medical interventions.

  • Assess the medical needs of patients upon intake/screening admission and residency.
  • Work with treatment team to develop the medical plan of action as reflected in the Treatment Plan
  • Evaluate patient’s daily overall health needs and report any medical needed information to medical and/or clinical supervisors.
  • Make medical recommendations and referrals as needed
  • Maintain all medical/ nursing records in compliance with OASAS regulations and Credo Community Center policy.
  • Insure all prescribed and over-the-counter medications are secured, inventoried, administered and recorded in compliance with agency policies and procedures.
  • Coordinate the scheduling of all patient medical care (e.g., dentist, physicals, hospital visits, etc.).
  • Provide emergency first – aid and determine if additional medical intervention is necessary.
  • Engage in clinical programming as directed.
  • Carry out treatment planning
  • Manage a case load
  • Facilitate groups
  • Maintain client records per OASAS standards and agency policy and procedures.
  • Conduct ongoing health related in – service trainings and classes including but not limited to:
  • -STD’s
    -Nutrition
    -Birth Control
    -Personal Hygiene
    -Cold/Hot weather related health issues

  • Maintain the medication area to meet program needs.
  • Interact with community resources regarding health related issues.
  • Serve on Quality Assurance committees as requested.
  • May serve as Health Coordinator
  • Provide support as needed for medical related services
  • Obtain breath and urine samples for AOD testing
  • Complete and process intake of clients
  • Administer TB tests to staff
  • Maintain Standards that apply to Confidentiality and 42 CFR/HIPAA
  • Will be required to perform other duties as requested, directed or assigned.

Minimum Job Qualifications:

Must be licensed as a Licensed Practical Nurse in New York State preferably with experience in the Chemical Dependency and/or Mental Health field.

And

Support the goal of abstinence from mood altering substances unless prescribed by a physician knowledgeable of addiction

And

Must subscribe to a Professional Canon of Ethics.
